Therapeutic effect and safety of eustachian tube surgery under nasal endoscope in the treatment of secretory otitis media

  •   Objective  To investigate the efficacy and superiority of triamcinolone acetonide and ambroxol injection via tympanic cavity in the treatment of secretory otitis media in the early stage of radiotherapy of nasopharyngeal carcinoma.
      Methods  Sixty patients with nasopharyngeal carcinoma, who had secretory otitis media and were found during clinical treatment, were enrolled. They were divided into surgery group(n=30) and drug group(n=30) according to the digital table method. The operation group was given traditional indwelling tube therapy after myringotomy, and the drug group was treated with triamcinolone acetonide and ambroxol injection via tympanic cavity. The therapeutic effects of the two groups were compared.
      Results  After treatment, the total effective rate of hearing in the drug group was significantly better than that in the surgery group (96.67% vs. 76.67%, P < 0.05). The hearing threshold of the two groups was significantly lower than treatment before (P < 0.05); the acoustic impedance test showed that ear canal peak suppression (MEP), static acoustic value (SC) and external auditory volume were significantly higher than normal values(P < 0.05). After treatment, the incidence of mucosal congestion, sinusitis, oral white mucosa and thick bloody discharge in the drug group were 16.67%, 23.33%, 36.67%, 20.00%, respectively, which were significantly lower than 50.00%, 63.33%, 66.67% and 43.33%, respectively, in the surgery group (P < 0.05).
      Conclusion  Triamcinolone acetonide and ambroxol injection via tympanic cavity has better therapeutic effect and is safer than traditional indwelling tube therapy after myringotomy.
