脑血栓患者血清Dickkopf相关蛋白1、骨硬化蛋白表达水平及其与疾病严重程度的关系

Expression levels of Dickkopf-related protein 1 and sclerostin protein in serum of patients with cerebral thrombosis and their relationships with disease severity

    目的 探讨脑血栓患者血清中Dickkopf相关蛋白1(DKK-1)、骨硬化蛋白(SOST)水平与疾病严重程度的关系。
    方法 选取110例脑血栓患者为脑血栓组, 并根据严重程度分为轻、中、重度组; 另选取同期一般资料相匹配的健康体检者110例为对照组。检测血清中DKK-1、SOST水平变化, 并进行相关性分析; 采用受试者工作特征(ROC)曲线分析DKK-1、SOST诊断重度脑血栓的效能。
    结果 脑血栓组血清DKK-1、SOST水平高于对照组, 差异有统计学意义(P < 0.05);轻度组、中度组、重度组血清中DKK-1、SOST的水平呈显著升高趋势(P < 0.05)。血清DKK-1与SOST水平存在正相关性(r=0.705, P < 0.05), 血清DKK-1、SOST水平均与脑血栓患者美国国立卫生研究院卒中量表(NIHSS)评分呈正相关性(r=0.625, r=0.674, P < 0.05)。DKK-1、SOST诊断重度脑血栓的曲线下面积(AUC)分别为0.879(95%CI: 0.808~0.950)、0.815(95%CI: 0.710~0.919);DKK-1联合SOST诊断的AUC为0.911(95%CI: 0.849~0.973)。
    结论 脑血栓患者血清中DKK-1、SOST表达水平增高, 且与疾病严重程度密切相关。DKK-1联合SOST对重度脑血栓有较高的诊断效能, 对二者水平进行监测有助于评估患者疾病严重程度。

     

    Abstract:
    Objective To explore the relationships of levels of Dickkopf-related protein 1(DKK-1) and sclerostin (SOST) in serum with disease severity in patients with cerebral thrombosis.
    Methods A total of 110 patients with cerebral thrombosis were selected as cerebral thrombosis group, and they were divided into mild, moderate and severe groups according to their severity; in addition, 110 healthy subjects with matching general data were selected as control group.Serum DKK-1 and SOST levels were detected and their correlation was analyzed.Receiver operator characteristic (ROC) curve was used to analyze the efficacy of DKK-1 and SOST in diagnosing severe cerebral thrombosis.
    Results The levels of DKK-1 and SOST in the cerebral thrombosis group were significantly higher the control group (P < 0.05);the serum levels of DKK-1 and SOST in severe, moderate and mild groups showed an significantly increasing trend (P < 0.05).There was a positive correlation between serum DKK-1 and SOST level (r=0.705, P < 0.05);the serum DKK-1 and SOST levels were positively correlated with National Institutes of Health Stroke Scale (NIHSS) score of patients with cerebral thrombosis (r=0.625, r=0.674, P < 0.05).The area under the curve (AUC) of DKK-1 and SOST in the diagnosis of severe cerebral thrombosis was 0.879(95%CI, 0.808 to 0.950) and 0.815(95%CI, 0.710 to 0.919), respectively; the AUC of DKK-1 combined with SOST was 0.911(95%CI, 0.849 to 0.973).
    Conclusion The levels of serum DKK-1 and SOST in patients with cerebral thrombosis are high and closely related to the severity of the disease.DKK-1 combined with SOST has a high diagnostic efficiency for severe cerebral thrombosis.Monitoring the levels of DKK-1 and SOST is helpful to evaluate the severity of the patient′s disease.
