内镜黏膜下剥离术治疗大肠巨大宽蒂息肉的临床分析

      目的  探讨内镜黏膜下剥离术(ESD)治疗大肠巨大宽蒂息肉的有效性、安全性及价值。
      方法  选取2015年1月—2019年5月江苏省武进人民医院和江苏省常州市中医医院收治的40例大肠巨大宽蒂息肉患者为研究对象，按不同治疗方法分为观察组和对照组。其中采用ESD治疗的20例患者为观察组，非ESD治疗的20例患者为对照组。比较2组患者术中出血率、术后1周迟发性出血率、病灶一次完整切除率以及操作时间。
      结果  观察组术中出血率较对照组低，观察组病灶一次完整切除率高于对照组，平均操作时间短于对照组，差异有统计学意义(P<0.05)。
      结论  ESD治疗巨大宽蒂息肉具有效果确切、创伤小、术中术后出血率低等优势。

     

    Abstract:
      Objective  To analyze the efficacy, safety and value of endoscopic submucosal dissection (ESD) in the treatment of giant wide-pedicle polyps in large intestine.
      Methods  A total of 50 patients with giant wide-pedicled polyp in large intestine in Wujin Hospital in Jiangsu Province and Changzhou Hospital of Traditional Chinese Medicine from January 2015 to May 2019 were divided into observation group and control group according to the varied treatment methods. The observation group(n=20) was treated with ESD, while the control group (n=20) was given non-ESD treatment. The intraoperative bleeding rate, delayed bleeding rate at one week after operation, complete rate for one-time resection and operative time were compared between the two groups.
      Results  The intraoperative bleeding rate in the observation group was lower than those in the control group(P<0.05). The complete rate for one-time resection in the observation group was higher, and the average operation time in the observation group was shorter than that in the control group.
      Conclusion  ESD has the advantages of accurate treatment, small trauma and low intraoperative bleeding rate in the treatment of wide-pedicle polyps.
