Research report on community chronic disease classification based on mortality

  • Objective To establish a "five-color" classification standard for enhanced management of community key people with hypertension in order to improve the efficiency of health management for chronic disease. Methods Based on the risk of death, a "five-color" classification standard for health management of patients with hypertension was constructed. Through the comparisons of the mortality between hypertension patients complicated with different chronic diseases, the different levels of risk factors and population risk ranks were determined. Results According to the level of mortality and the number of cases with different disease combinations, the community hypertension patients were divided into five risk levels on the basis of risk factors superposition, including Purple, Red, Orange, Yellow and Green, and the corresponding mortality rates were 16.92%, 14.77%, 10.33%, 4.15% and 1.53% respectively. The proportions of hypertension patients matched with "five-color" classification standard in the population were 0.62%, 0.73%, 2.02%, 21.81% and 74.83% respectively. Conclusion The "five-color" classification standard is feasible and simple in operation, which can realize the hierarchy of risk management and the controllability of population size.
