Objective To explore a role of nurse-led chronic disease management strategy on controlling of blood glucose in patients with type 2 diabetes (T2DM).
Methods A total of 69 T2DM patients were recruited and divided into study group(n=38) and control group(n=31). Glycosylated hemoglobin (HbA1c), blood pressure, blood lipid and treatment compliance were observed after 6 months of nurse-led chronic disease management strategy.
Results After 6 intervention, the decrease degree of HbA1c, improvement of systolic blood pressure as well as triglyceride and treatment compliance in the study group were significantly better than that in the control group, which showed significant differences(P < 0.05).
Conclusion A nurse-ledchronic disease management strategy can significantly control the blood glucose, improve treatment compliance in patients with type 2 diabetes, and is benefit in effective management of patients with diabetes.
