耳穴揿针联合右佐匹克隆片治疗脑卒中后失眠患者的疗效及对血清脑源性神经营养因子、白细胞介素-1β水平的影响

Effect of auricular plaster therapy with press needles combined with eszopiclone tablets and their impacts on serum levels of brain-derived neurotrophic factor and interleukin-1β in patients with post-stroke insomnia

    目的 探究耳穴揿针联合右佐匹克隆片治疗脑卒中后失眠(PSI)患者的疗效及对血清脑源性神经营养因子(BDNF)和白细胞介素(IL)-1β水平的影响。
    方法 选取120例PSI患者为研究对象,随机将其分为A组(60例)和B组(60例)。A组给予右佐匹克隆片治疗, B组采用耳穴揿针联合右佐匹克隆片治疗, 2组患者均连续治疗4周。比较2组治疗后临床疗效、睡眠情况、炎症因子、神经营养因子、心理状态、生活质量、不良反应情况。
    结果 治疗后, B组临床总有效率为93.33%, 高于A组的80.00%, 差异有统计学意义(P < 0.05)。2组患者的肿瘤坏死因子-α(TNF-α)、IL-1β、IL-6、去甲肾上腺素(NE)水平、匹兹堡睡眠质量指数(PSQI)评分、汉密尔顿抑郁量表(HAMD)评分、汉密尔顿焦虑量表(HAMA)评分以及睡眠状况自评量表(SRSS)评分均较治疗前降低,且B组以上指标均低于A组,差异有统计学意义(P < 0.05)。2组患者的5-羟色胺(5-HT)、BDNF水平和脑卒中专用生活质量量表(SS-QOL)评分较治疗前提高,且B组高于A组,差异有统计学意义(P < 0.05)。2组患者不良反应总发生率比较,差异无统计学意义(P>0.05)。
    结论 耳穴揿针联合右佐匹克隆片可提高PSI患者临床疗效,改善睡眠和生活质量,降低BDNF和IL-1β水平,减轻焦虑和抑郁情绪,具有较高的安全性。

     

    Abstract:
    Objective To explore the efficacy of auricular plaster therapy with press needles combined with eszopiclone tablets in the treatment of patients with post-stroke insomnia (PSI) and its effect on the serum levels of brain-derived neurotrophic factor (BDNF) and interleukin-1β (IL-1β).
    Methods A total of 120 PSI patients were selected as the study subjects and randomly divided into group A (60 cases) and group B (60 cases). Group A was treated with eszopiclone tablets, while group B was treated with auricular plaster therapy with press needles combined with eszopiclone tablets. Both groups received continuous treatment for 4 weeks. The clinical efficacy, sleep status, inflammatory factors, neurotrophic factors, psychological state, quality of life, and adverse reactions were compared between the two groups after treatment.
    Results After treatment, the total clinical effective rate in the group B was 93.33%, which was higher than 80.00% in the group A (P < 0.05). The levels of tumor necrosis factor-α (TNF-α), IL-1β, IL-6, norepinephrine (NE), the scores of Pittsburgh Sleep Quality Index (PSQI), Hamilton Depression Scale (HAMD), Hamilton Anxiety Scale (HAMA), and Self-rating Scale of Sleep (SRSS) in both groups were lower than those before treatment (P < 0.05), and the above indicators in the group B were lower than those in group A, with statistically significant differences (P < 0.05). The levels of 5-hydroxytryptamine (5-HT), BDNF, and the scores of Stroke-Specific Quality of Life Scale (SS-QOL) were higher than those before treatment, and the levels in the group B were higher than those in the group A, with statistically significant differences (P < 0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups (P>0.05).
    Conclusion Auricular plaster therapy with press needles combined with eszopiclone tablets can improve the clinical efficacy, sleep status, and quality of life in PSI patients, reduce the levels of BDNF and IL-1β, alleviate anxiety and depression, and have high safety.
