桃红四物汤加减联合那屈肝素对髋关节置换术患者血小板活化指标及血液流变学的影响

Effect of modified Taohong Siwu Decoction combined with nadroparin on platelet activation markers and hemorheology in patients undergoing hip replacement

    目的 探讨桃红四物汤加减联合那屈肝素预防髋关节置换术后下肢深静脉血栓(DVT)的效果。
    方法 将85例髋关节置换术患者随机分为实验组43例和常规组42例。常规组术后给予那屈肝素,实验组给予桃红四物汤加减联合那屈肝素。比较2组DVT发生率、患肢肿胀阳性率、手术前后凝血因子D-二聚体(D-D)、纤维蛋白原降解产物(FDP)、纤溶酶-α2纤溶酶抑制物复合物(PIC)、凝血酶-抗凝血酶Ⅲ复合物(TAT)、血小板活化指标膜糖蛋白Ⅱb/Ⅲa(GP Ⅱb/Ⅲa)、P-选择素(CD62P)、颗粒膜蛋白140(GMP-140)、血小板膜糖蛋白纤维蛋白原受体-1(PAC-1)、血液流变学指标(血浆黏度、全血低切黏度、全血中切黏度、全血高切黏度)及出血事件发生情况。
    结果 实验组DVT发生率、患肢肿胀阳性率低于常规组,差异有统计学意义(P < 0.05)。治疗14 d后,实验组血浆D-D、FDP、TAT、PIC、GP Ⅱb/Ⅲa、CD62P、GMP-140、PAC-1以及全血高切黏度、全血中切黏度、全血低切黏度、血浆黏度均低于常规组,差异均有统计学意义(P < 0.05)。2组治疗期间均未发生出血事件。
    结论 桃红四物汤加减联合那屈肝素能改善髋关节置换术后患者血栓分子与血小板状态,降低下肢DVT风险,减轻肿胀。

     

    Abstract:
    Objective To investigate the effect of modified Taohong Siwu Decoction combined with nadroparin in preventing deep venous thrombosis (DVT) of the lower extremities after hip replacement.
    Methods A total of 85 patients undergoing hip replacement were randomly divided into experimental group (n=43) and conventional group (n=42). The conventional group received nadroparin postoperatively, while the experimental group received modified Taohong Siwu Decoction combined with nadroparin. The incidence of DVT, positive rate of limb swelling, coagulation factors D-dimer (D-D), fibrinogen degradation products (FDP), plasmin-α2 plasmin inhibitor complex (PIC), thrombin-antithrombin III complex (TAT), platelet activation markers glycoprotein Ⅱb/Ⅲa (GP Ⅱb/Ⅲa), P-selectin (CD62P), granule membrane protein 140 (GMP-140), platelet glycoprotein fibrinogen receptor-1 (PAC-1), hemorheological indicators (plasma viscosity, low-shear whole blood viscosity, medium-shear whole blood viscosity, high-shear whole blood viscosity), and the occurrence of bleeding events were compared between the two groups before and after surgery.
    Results The incidence of DVT and positive rate of affected limb swelling were lower in the experimental group than those in the conventional group(P < 0.05). Fourteen days after treatment, the plasma levels of D-D, FDP, TAT, PIC, GP Ⅱb/Ⅲa, CD62P, GMP-140, PAC-1, as well as high-shear whole blood viscosity, medium-shear whole blood viscosity, low-shear whole blood viscosity, and plasma viscosity were all lower in the experimental group than those in the conventional group (P < 0.05). No bleeding events occurred in either group during treatment.
    Conclusion Modified Taohong Siwu Decoction combined with nadroparin can improve the thrombin and platelet status in patients after hip replacement, restore blood microcirculation, reduce the risk of DVT in the lower extremities, and alleviate swelling.
