肘关节前内侧入路治疗尺骨冠状突O′Driscoll Ⅱ型和Ⅲ型骨折的疗效观察

Effect of anteromedial approach of elbow joint in treating O′Driscoll type Ⅱ and Ⅲ fractures of coronoid process of ulna

    目的 观察肘关节前内侧入路治疗尺骨冠状突O′Driscoll Ⅱ型和Ⅲ型骨折的疗效。
    方法 选取100例尺骨冠状突O′Driscoll Ⅱ型和Ⅲ型骨折患者为研究对象,按照入院先后顺序分为对照组和研究组,每组50例。对照组给予外侧入路治疗,研究组给予肘关节前内侧入路治疗。比较2组疗效、肘关节活动度及并发症发生情况。
    结果 研究组优良率为90.00%, 高于对照组的74.00%, 差异有统计学意义(P < 0.05); 术后1、3、6个月,研究组肘关节屈伸活动度分别为(88.69±11.32)°、(111.14±14.02)°、(121.21±15.34)°, 高于对照组的(76.16±11.57)°、(98.33±12.27)°、(110.38±13.80)°, 研究组肘关节旋转活动度分别为(91.29±10.16)°、(111.29±16.28)°、(121.47±16.39)°, 高于对照组的(79.85±10.69)°、(100.61±13.30)°、(113.46±12.62)°, 差异均有统计学意义(P < 0.05); 研究组术后并发症发生率为10.00%, 低于对照组的28.00%, 差异有统计学意义(P < 0.05)。
    结论 肘关节前内侧入路治疗尺骨冠状突O′Driscoll Ⅱ型和Ⅲ型骨折患者具有较好的疗效,有利于肘关节功能恢复,且安全性好。

     

    Abstract:
    Objective To observe the efficacy of anteromedial approach of elbow joint in treating O′Driscoll type Ⅱ and Ⅲ fractures of coronoid process of ulna.
    Methods A total of 100 patients with O′Driscoll type Ⅱ and Ⅲ fractures of coronoid process of ulna were selected as research objects, and they were divided into control group and study group according to the order of hospital admission, with 50 cases in each group. The control group was treated with a lateral approach, while the study group was treated with an anteromedial approach of elbow joint. The efficacy, mobility of elbow joint and incidence of complications were compared between the two groups.
    Results The excellent rate of the study group was 90.00%, which was significantly higher than 74.00% of the control group (P < 0.05); at 1 month, 3 and 6 months after surgery, the flexion and extension mobility of elbow joint in the study group was (88.69±11.32)°, (111.14±14.02)° and (121.21±15.34)° respectively, which was significantly higher than (76.16±11.57)°, (98.33±12.27)° and (110.38±13.80)° in the control group (P < 0.05); the rotation mobility of elbow joint in the study group was (91.29± 10.16)°, (111.29±16.28)° and (121.47±16.39)° respectively, which was significantly higher than (79.85±10.69)°, (100.61±13.30)° and (113.46±12.62)° in the control group (P < 0.05); the postoperative incidence rate of complications in the study group was 10.00%, which was significantly lower than 28.00% in the control group (P < 0.05).
    Conclusion Anteromedial approach of elbow joint is effective in treating patients with O′Driscoll type Ⅱ and Ⅲ fractures of coronoid process of ulna, which is beneficial for the recovery of elbow joint function, and has higher safety.
