直肠癌患者治疗前后循环肿瘤DNA、癌胚抗原、糖类抗原199水平变化及其相关性

Changes of circulating tumor DNA, carcinoembryonic antigen and carbohydrate antigen 199 levels in patients with colorectal cancer before and after treatment and their correlations

    目的 探讨直肠癌(CRC)患者治疗前后循环肿瘤DNA (ctDNA)、癌胚抗原(CEA)和糖类抗原199(CA199)水平变化及其相关性。
    方法 选取38例CRC患者, 采用放疗联合口服卡培他滨治疗5周。检测并分析所有患者治疗前后ctDNA、CEA和CA199水平变化。分析ctDNA与CEA、CA199之间的相关性。
    结果 治疗后患者血清CA199、CEA、ctDNA水平低于治疗前,差异有统计学意义(P < 0.05)。治疗前, ctDNA水平与CEA无相关性(r=0.005, P=0.323), 与CA199水平呈正相关(r=0.365, P=0.021)。治疗后, ctDNA水平与CEA无相关性(r=-0.013, P=0.354), 与CA199水平呈正相关(r=0.318, P=0.013)。
    结论 相比于CEA水平变化, ctDNA、CA199水平变化对CRC患者临床疗效、预后评估的价值更高。

     

    Abstract:
    Objective To investigate the changes of circulating tumor DNA (ctDNA), carcinoembryonic antigen (CEA)and carbohydrate antigen 199 levels (CA199) in patients with colorectal cancer (CRC) before and after treatment and their correlations.
    Methods Thirty-eight CRC patients were selected, and were treated with radiotherapy combined with oral capecitabine for 5 weeks. The changes of ctDNA, CEA and CA199 levels before and after treatment were detected and analyzed. The correlations of ctDNA with CEA and CA199 were analyzed.
    Results The levels of serum CA199, CEA and ctDNA after treatment were significantly lower than those before treatment (P < 0.05). Before treatment, ctDNA level was not correlated with CEA (r=0.005, P=0.323), and positively correlated with CA199 (r=0.365, P=0.021). After treatment, ctDNA level was not correlated with CEA level (r=-0.013, P=0.354), and positively correlated with CA199 level (r=0.318, P=0.013).
    Conclusion Compared with change in CEA level, changes in ctDNA and CA199 levels are more valuable in evaluating the clinical efficacy and prognosis of CRC patients.
