Effect of the duration of microscopic apical surgery on postoperative pain

    目的 探讨显微根尖手术时间对患者术后疼痛程度的影响, 并分析改良护理措施减轻术后疼痛的可行性。
    方法 回顾性分析上海市第九人民医院牙体牙髓科2021年5月—2022年2月治疗的109例显微根尖手术患者的临床资料,依据手术时间不同将患者分为A组(手术时间 < 1.0 h)、B组(手术时间在1.0~1.5 h)、C组(手术时间>1.5~2.0 h)以及D组(手术时间>2.0 h), 并对其术后疼痛程度采用数字评定量表(NRS)评分进行评价,分析显微根尖手术时间对患者术后疼痛程度的影响。
    结果 术后第1天至第6天, B组、C组和D组与A组NRS评分比较,差异有统计学意义(P < 0.05); C组术后第7天至第10天的NRS评分与A组比较,差异有统计学意义(P < 0.05); D组术后第7天NRS评分与A组比较,差异有统计学意义(P < 0.05)。C组术后第1天至第10天NRS评分及D组术后第1天至第7天NRS评分与B组比较,差异有统计学意义(P < 0.05)。C组与D组不同时点NRS评分比较,差异无统计学意义(P>0.05)。
    结论 显微根尖手术时间越短,术后疼痛程度越轻,恢复时间越短,因此缩短手术时间能改善患者术后疼痛程度。

     

    Abstract:
    Objective To investigate the effect of microscopic root apical surgery time on postoperative pain degree of patients, and analyze the feasibility of improved nursing measures in reducing postoperative pain.
    Methods The clinical data of 109 patients who underwent microapical surgery in the Department of Dentistry and Endodontics of Shanghai Ninth People's Hospital from May 2021 to February 2022 were retrospectively analyzed. The patients were divided into group A (operation time < 1.0 h), group B (operation time 1.0 to 1.5 h), group C (operation time> 1.5 to 2.0 h) and group D (operation time>2.0 h) according to the duration of surgery, and their postoperative pain was evaluated by numerical rating scale(NRS). The effect of microapical operation time on postoperative pain was analyzed.
    Results From the first day after surgery to the sixth day after operation, the NRS scores of groups B, C and D showed significant difference compared with those of group A (P < 0.05). The NRS scores of group C from the postoperative seventh day to the tenth day showed significant difference compared with those of group A (P < 0.05). The NRS score of group D on the 7th day after operation showed significant difference compared with that of group A (P < 0.05). The NRS score of group C from the postoperative first day to the tenth day after operation and the NRS score of the first day to seventh day after operation showed significant differences compared with those of group B (P < 0.05). There were no significant differences in NRS scores between group C and group D at different time points (P>0.05).
    Conclusion The shorter the microscopic apical surgery time is, the less the postoperative pain degree and the shorter the recovery time will be. Therefore, shortening the operation time can improve the postoperative pain degree of patients.
