左氧氟沙星与加替沙星对白内障围术期患者康复效果的比较

Levofloxacin versus gatifloxacin in rehabilitation effect of patients with cataract during perioperative period

    目的  比较左氧氟沙星与加替沙星对白内障围术期患者结膜囊细菌阳性结果、角膜荧光素染色(FLS)评分、生活满意度量表(SWLS)评分的影响。
    方法  选取2019年5月—2021年6月104例接受白内障手术治疗患者为研究对象,随机分为观察组(给予加替沙星治疗)与对照组(给予左氧氟沙星治疗),每组52例。比较2组术前使用抗菌药点眼3 d后(T1)、术后使用抗菌药点眼7 d后(T2)的细菌培养阳性率; 比较2组临床症状评分、FLS评分、SWLS评分、炎症因子白细胞介素-8(IL-8)、可溶性细胞间黏附分子-1(sICAM-1)、肿瘤坏死因子-α(TNF-α)水平以及不良反应发生率。
    结果  观察组T1、T2时的细菌培养阳性率分别为25.00%(13/52)、1.92%(1/52), 低于对照组的44.23%(23/52)、15.38%(8/52), 差异有统计学意义(P < 0.05)。治疗后,观察组临床症状评分、FLS评分以及IL-8、sICAM-1、TNF-α水平低于对照组, SWLS评分高于对照组,差异均有统计学意义(P < 0.05)。观察组不良反应发生率为3.85%, 低于对照组的26.92%, 差异有统计学意义(P < 0.05)。
    结论  加替沙星的抗菌效果优于左氧氟沙星,能降低细菌培养阳性率,改善临床症状,且安全性较好。

     

    Abstract:
    Objective  To compare the influence of levofloxacin and gatifloxacin on the positive result of conjunctival sac bacteria, corneal fluorescein staining (FLS) score and the Satisfaction with Life Scale (SWLS) score in patients with cataract during perioperative period.
    Methods  A total of 104 patients with cataract surgery from May 2019 to June 2021 were selected as research objects, and were randomly divided into observation group (treated with gatifloxacin) and control group (treated with levofloxacin), with 52 cases in each group. Positive rate of bacterial culture was compared between two group at the time points of 3 days after dripping antibacterial ophthalmic before surgery (T1) and 7 days after dripping antibacterial ophthalmic after operation (T2); the score of clinical symptoms, FLS score, SWLS score, levels of inflammatory factorsinterleukin-8 (IL-8), soluble intercellular adhesion molecule-1 (sICAM-1), tumor necrosis factor-α (TNF-α)and incidence of adverse reactions were compared between two groups.
    Results  The positive rates of bacterial culture at T1 and T2 in the observation group were 25.00%(13/52) and 1.92%(1/52) respectively, which were significantly lower than 44.23%(23/52) and 15.38%(8/52) in the control group (P < 0.05). After treatment, the score of clinical symptoms, FLS score and levels of IL-8, sICAM-1 and TNF-α in the observation group were significantly lower than those in the control group, while the SWLS score was significantly higher than that in the control group (P < 0.05). The incidence of adversereactions in the observation group was 3.85%, which was significantly lower than 26.92% in the control group (P < 0.05).
    Conclusion  Gatifloxacin is better than levofloxacinin in antibacterial effect, which can reduce the positive rate of bacterial culture, improve clinical symptoms, and has higher safety.
