血清同型半胱氨酸、可溶性白介素2受体水平与急性胰腺炎患者病情严重程度的关系

Relationships of serum levels of homocysteine, serum soluble interleukin 2 receptor with severity of illness in patients with acute pancreatitis

    目的 探讨血清同型半胱氨酸(Hcy)、可溶性白介素2受体(sIL-2R)水平与急性胰腺炎(AP)患者急性生理功能和慢性健康状况评分系统Ⅱ(APACHE Ⅱ)评分的关系。
    方法 选取AP患者82例纳入AP组,另选取同期健康体检者66例纳入对照组,比较2组血清Hcy、sIL-2R水平。根据APACHE Ⅱ评分将82例AP患者分为轻症AP(MAP)组(APACHE Ⅱ评分 < 8分)50例和重症AP(SAP)组(APACHE Ⅱ评分≥8分)32例,比较2组血清Hcy、sIL-2R水平和APACHE Ⅱ评分。采用Pearson相关分析法分析AP患者血清Hcy、sIL-2R水平与APACHE Ⅱ评分的相关性。
    结果 AP组血清Hcy、sIL-2R水平依次为(10.75±2.05) mmol/L、(39.46±8.12) pg/mL, 分别高于对照组的(6.02±0.37) mmol/L、(14.51±3.64) pg/mL, 差异有统计学意义(P < 0.05)。SAP组血清Hcy、sIL-2R水平和APACHE Ⅱ评分依次为(15.93±0.37) mmol/L、(51.32±11.09) pg/mL和(16.74±3.11)分,分别高于MAP组的(7.44±1.12) mmol/L、(31.87±6.58) pg/mL和(4.11±0.64)分,差异有统计学意义(P < 0.05)。相关性分析结果显示,血清Hcy、sIL-2R水平均与APACHE Ⅱ评分呈正相关(P < 0.05)。
    结论 血清Hcy、sIL-2R在AP患者中呈高表达,且与APACHE Ⅱ评分具有一定相关性,有望成为预测AP患者病情严重程度的辅助指标。

     

    Abstract:
    Objective To explore the relationships of serum homocysteine (Hcy), serum soluble interleukin 2 receptor (SIL-2R) levels with Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ) scores in acute pancreatitis (AP) patients.
    Methods A total of 82 AP patients were selected as AP group, and 66 healthy subjects were selected as control group. Serum levels of Hcy and sIL-2R were compared between the two groups. According to APACHE Ⅱ score, they were divided into mild AP group (MAP group, n=50, APACHE Ⅱ score < 8), severe AP group (SAP group, n=32, APACHE Ⅱ score ≥ 8). The correlations of Hcy and SIL-2R with APACHE Ⅱ score were analyzed by Pearson method.
    Results The levels of serum Hcy and SIL-2R in the AP group were (10.75±2.05) mmol/L, (39.46±8.12) pg/mL, respectively, which were significantly higher than (6.02±0.37) mmol/L and (14.51±3.64) pg/mL in the control group (P < 0.05). Serum Hcy and SIL-2R levels and APACHE Ⅱ score in the SAP group were (15.93±0.37) mmol/L (51.32±11.09) pg/mL and (16.74±3.11) points, which were higher than (7.44±1.12) mmol/L, (31.87±6.58) pg/mL and (4.11±0.64) points in the MAP group (P < 0.05). Correlation analysis showed that serum Hcy, sIL-2R were positively correlated with APACHE Ⅱ score (P < 0.05).
    Conclusion Serum Hcy and sIL-2R are highly expressed in AP patients, and have a certain correlation with the APACHE Ⅱ score, which are expected to become auxiliary indicators for predicting the severity of AP patients.
