重型颅脑损伤患者血浆外泌体微小RNA-202-5p表达水平及其对预后的预测价值

Expression of plasma exosomal microRNA-202-5p in patients with severe traumatic brain injury and its value in predicting prognosis

    目的  观察重型颅脑损伤(sTBI)患者血浆外泌体微小RNA-202-5p(miR-202-5p)的表达水平及其对预后的预测价值。
    方法  选择sTBI患者78例为研究对象,根据格拉斯哥预后量表(GOS)评分将sTBI患者分为预后不良(n=27)和预后良好(n=51)患者,同时选择同期50例健康人群为对照。提取人群血浆外泌体,检测外泌体miR-202-5p表达水平,采用Logistic回归分析探讨预后不良的相关因素,采用受试者工作特征(ROC)曲线分析miR-202-5p对预后的预测价值。
    结果  sTBI患者血浆外泌体miR-202-5p表达水平为(0.8±0.1), 低于健康人群的(3.7±0.2), 差异有统计学意义(t=12.235, P < 0.01); 预后不良患者格拉斯哥昏迷量表(GCS)评分、miR-202-5p水平及鹿特丹CT评分低于预后良好患者,急性生理学和慢性健康状况评价Ⅱ(APACHE Ⅱ)评分高于预后良好患者,差异均有统计学意义(P < 0.05); GCS评分、miR-202-5p水平、APACHE Ⅱ评分及鹿特丹CT评分是影响患者预后的独立危险因素; 血浆外泌体miR-202-5p水平与鹿特丹CT评分和GCS评分呈正相关(r=0.703、0.711, P < 0.01), 与APACHE Ⅱ评分呈负相关(r=-0.693, P < 0.01); miR-202-5p的ROC曲线下面积(AUC)预测患者预后的效果优于APACHE Ⅱ评分、鹿特丹CT评分及GCS评分。
    结论  sTBI患者血浆外泌体低表达miR-202-5p能够反映疾病预后。

     

    Abstract:
    Objective  To observe the expression level of plasma exosomal microRNA-202-5p (miR-202-5p) in patients with severe traumatic brain injury (sTBI) and its value in predicting prognosis.
    Methods  A total of 78 patients with sTBI were selected as study objects, and were divided into poor prognosis group and good prognosis group according to Glasgow Prognosis Scale (GOS) score, and 50 normal people in the same period were selected as control group. The plasma exosomes were extracted, and the expression level of miR-202-5p was detected. Logistic regression analysis was used to explore the factors associated with poor prognosis, and receiver operating characteristic (ROC) curve was used to analyze the prognostic value of miR-202-5p.
    Results  The expression level of plasma exosomal miR-202-5p of STBI patients was (0.84±0.09), which was lower than (3.72±0.16) of the normal subjects (t=12.235, P < 0.01); the Glasgow Coma Scale (GCS) score, miR-202-5p level and Rotterdam CT score of patients with poor prognosis were lower than those of patients with good prognosis, and the Acute Physiology and Chronic Health Status Ⅱ (APACHE Ⅱ) score was higher than that of patients with good prognosis (P < 0.05). GCS score, miR-202-5p level, APACHE Ⅱ score and Rotterdam CT score were independent risk factors affecting the prognosis of patients. The level of miR-202-5p was positively correlated with Rotterdam CT score and GCS score (r= 0.703, 0.711, P < 0.01), and negatively correlated with APACHE Ⅱscore (r=-0.693, P < 0.01). The effect of area under ROC curve (AUC) of miR-202-5p in predicting prognosis of patients was better than that of APACHE Ⅱ score, Rotterdam CT score and GCS score.
    Conclusion  The low expression of plasma exosomal miR-202-5p of sTBI patients can reflect the prognosis of the disease.
