ω-3鱼油脂肪乳对胃癌患者术后肝肾功能及预后的影响

Effect of ω-3 fish oil fat emulsion on postoperative liver and renal functions and prognosis in patients with gastric cancer

      目的  探讨ω-3鱼油脂肪乳对胃癌患者术后肝肾功能及预后的影响。
      方法  采用回顾性队列研究方法,收集南京市第一医院普外科2018年1月-2019年12月126例胃癌术后患者的资料,并将患者随机分为对照组(肠外营养中应用常规脂肪乳)和观察组(对照组基础上加用ω-3鱼油脂肪乳),每组63例。比较2组患者术前1 d和术后7 d的肝肾功能指标谷氨酸-丙酮酸转氨酶(ALT)、天门冬氨酸转氨酶(AST)、总胆红素(TBIL)、肌酐(Cr)、血尿素氮(BUN)、营养指标总蛋白(TP)和白蛋白(ALB)和血脂指标甘油三酯(TG)和总胆固醇(TC)。比较2组住院时间、术后住院时间、术后并发症和术后通气时间。
      结果  观察组术后7 d的ALT、TBIL和TG水平低于对照组,ALB高于对照组,差异有统计学意义(P < 0.05)。2组患者住院时间、术后住院时间、通气时间和术后并发症比较,差异无统计学意义(P>0.05)。
      结论  ω-3鱼油脂肪乳可改善患者肝功能、脂肪代谢,减少ALB降低幅度,但在缩短住院时间、减少并发症和改善肾功能方面未观察到明显的优势。

     

    Abstract:
      Objective  To explore the effect of ω-3 fish oil fat emulsion on postoperative liver and renal functions and prognosis in patients with gastric cancer.
      Methods  A retrospective cohort study was adopted, and the materials of 126 patients after gastric cancer surgery from January 2018 to December 2019 in the Department of General Surgery in the First Hospital of Nanjing City were collected. All the patients were randomly divided into control group (treated with routine fat emulsion in parenteral nutrition) and observation group (treated with additional ω-3 fish oil fat emulsion on the basis of control group), with 63 cases in each group. At the time points of one day before operation and 7 days after operation, the indexes of liver and renal functionsalanine transaminase (ALT), aspartate aminotransferase (AST), total bilirubin (TBIL), creatinine (Cr), blood urea nitrogen (BUN), nutritional indexestotal protein (TP) and albumin (ALB)and blood lipid indexestriglyceride (TG) and total cholesterol (TC)were compared between the two groups. The hospital stay, postoperative hospital stay, postoperative complications and postoperative ventilation time were compared between the two groups.
      Results  Levels of ALT, TBIL and TG at 7 days after operation in the observation group were significantly lower than those in the control group, while ALB level was significantly higher than that in the control group (P < 0.05). There were no significant differences in hospital stay, postoperative hospital stay, ventilation time and postoperative complications between the two groups (P>0.05).
      Conclusion  ω-3 fish oil fat emulsion can improve liver function and fat metabolism of patients, reduce the reduction range of ALB, but shows no obvious advantages in shortening hospital stay, reducing complications and improving renal function.
