老年亚临床甲状腺功能减退症合并脑梗死患者甲状腺功能及血脂水平分析

Analysis in thyroid functions and blood lipid levels in elderly patients with subclinical hypothyroidism complicated with cerebral infarction

      目的  分析60岁以上亚临床甲状腺功能减退症(简称甲减)合并脑梗死患者的甲状腺功能、血脂水平。
      方法  将84例亚临床甲减合并脑梗死患者纳入合并组,84例亚临床甲减患者纳入甲减组,84例脑梗死患者纳入脑梗死组,并将同期常规体检的84例健康老人纳入健康组,4组受试者年龄均大于60岁,比较4组受试者的甲状腺功能指标促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、血脂指标甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平差异。
      结果  合并组、甲减组的TSH水平高于脑梗死组、健康组,差异有统计学意义(P < 0.05)。合并组、甲减组、脑梗死组TG、TC、LDL-C水平高于健康组,合并组、甲减组TG、TC、LDL-C水平高于脑梗死组,合并组TG、TC、LDL-C水平高于甲减组,差异均有统计学意义(P < 0.05)。
      结论  60岁以上亚临床甲减合并脑梗死患者的甲状腺功能、血脂水平存在明显异常,临床应予以重视并及时治疗。

     

    Abstract:
      Objective  To analyze thyroid functions and blood lipid levels in patients with subclinical hypothyroidism and cerebral infarction aged more than 60 years.
      Methods  A total of 84 patients with subclinical hypothyroidism over 60 years old were included in combined group, 84 patients with subclinical hypothyroidism and cerebral infarction over 60 years old were included in hypothyroidism group, 84 patients with cerebral infarction over 60 years old were included in cerebral infarction group, and 84 healthy elderly people over 60 years old who underwent routine physical examination at the same time were included in healthy group. Level of thyroid function indexes thyroid-stimulating hormone (TSH), three free iodine thyroid former glycine (FT3) and free thyroxine (FT4) and blood lipids indexes triglyceride (TG), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C) of four groups were compared.
      Results  The levels of TSH in combined group and hypothyroidism group were higher than those in cerebral infarction group and healthy group (P < 0.05). The levels of TG, TC and LDL-C in the combined group, hypothyroidism group and cerebral infarction group were higher than those in the healthy group, the levels of TG, TC and LDL-C in the combined group and hypothyroidism group were higher than those in the cerebral infarction group, the levels of TG, TC and LDL-C in the combined group were higher than those in the hypothyroidism group (P < 0.05).
      Conclusion  The thyroid functions and blood lipid levels of patients over 60 years old with subclinical hypothyroidism complicated with cerebral infarction are obviously abnormal. Therefore, attention should be paid to their changes and treatment should be given in time.
