全数字化乳腺摄影联合数字乳腺断层摄影对乳腺病变及乳腺钙化的诊断价值

Value of full-field digital mammography combined with digital breast tomography in detection of breast lesions and breast calcification

      目的   探讨全数字化乳腺摄影(FFDM)、数字乳腺断层摄影(DBT)单独应用及联合应用对乳腺良恶性病变及乳腺钙化的诊断价值。
      方法   选取280例疑似乳腺病变患者作为研究对象,患者均接受FFDM、DBT检查,比较FFDM、DBT单独应用及联合应用对乳腺良恶性病变、乳腺钙化的诊断准确率,并采用受试者工作特征(ROC)曲线分析FFDM、DBT、FFDM联合DBT对乳腺病变的预测价值。
      结果   FFDM的阳性预测值为63.53%, 阴性预测值为85.21%; DBT的阳性预测值为78.75%, 阴性预测值为90.80%; DBT联合FFDM的阳性预测值为92.50%, 阴性预测值为97.13%。FFDM联合DBT对乳腺良恶性病变、乳腺钙化的诊断准确率高于DBT、FFDM, DBT对乳腺良恶性病变、乳腺钙化的诊断准确率高于FFDM, 差异有统计学意义(P < 0.05)。ROC曲线分析结果显示, FFDM、DBT、FFDM联合DBT早期预测乳腺病变的曲线下面积(AUC)分别为0.753、0.850、0.951。
      结论   相较于FFDM、DBT单独检查, DBT联合FFDM检查对乳腺病变的诊断价值更高,可有效分辨乳腺良恶性病变,并提高乳腺钙化的检出率。

     

    Abstract:
      Objective   To explore the diagnostic values of full-field digital mammography (FFDM) alone, digital breast tomography (DBT) alone and their combination in the diagnosis of benign or malignant breast lesions and breast calcification.
      Methods   A total of 280 patients with suspected breast lesions were selected as research objects, and all patients received FFDM and DBT examinations. The diagnostic accuracy of FFDM or DBT alone and their combination in the diagnosis of benign or malignant breast lesions and breast calcification was compared. Receiver operating characteristic (ROC) curve was used to analyze the predictive value of FFDM, DBT and FFDM combined with DBT for breast lesions.
      Results   The positive prediction value of FFDM was 63.53% and the negative prediction value was 85.21%; the positive prediction value of DBT was 78.75% and the negative prediction value was 90.80%; the positive prediction value of DBT combined with FFDM was 92.50%, and the negative prediction value was 97.13%. The accuracy rates of FFDM combined with DBT in diagnosis of benign, malignant breast lesions and breast calcification were higher than that of DBT and FFDM alone, and the accuracy rates of DBT in diagnosis of benign and malignant breast lesions and breast calcification were higher than those of FFDM (P < 0.05). ROC curve analysis showed that the area under curve (AUC) of FFDM, DBT and FFDM combined with DBT in early prediction of breast lesions were 0.753, 0.850 and 0.951, respectively.
      Conclusion   DBT combined with FFDM have higher diagnostic value for breast lesions, effectively distinguish benign and malignant lesions, and increase the detection rate of breast calcification.
