外周血LncRNA-p3134表达水平在妊娠期糖尿病诊断中的价值

Value of expression level of lncRNA-p3134 in diagnosing gestational diabetes mellitus

      目的  探讨外周血LncRNA-p3134表达水平在妊娠期糖尿病(GDM)诊断中的价值。
      方法  选取GDM孕妇82例为GDM组,非GDM孕妇116例为对照组。检测2组外周血LncRNA-p3134的表达水平;分析LncRNA-p3134表达与胰岛素抵抗指数(HOMA-IR)、胰岛素敏感性指数(ISOGTT)和胰岛β细胞功能指数(HOMA-β)水平、GDM发生的相关性;分析LncRNA-p3134表达对GDM发生的影响以及LncRNA-p3134在GDM诊断中的作用。
      结果  GDM组LncRNA-p3134相对表达水平高于对照组,差异有统计学意义(P < 0.05)。LncRNA-p3134表达与ISOGTT、HOMA-β呈负相关(P < 0.05),与HOMA-IR、GDM发生呈正相关(P < 0.05)。校正混杂因素后,LncRNA-p3134表达是GDM发生的独立影响因素(P < 0.05),LncRNA-p3134表达相对量越高则GDM发生风险越大。受试者工作特征(ROC)曲线分析显示,LncRNA-p3134诊断GDM的曲线下面积(AUC)为0.853,敏感度和特异度分别为75.0%、83.0%。
      结论  LncRNA-p3134在GDM中表达水平较非GDM高,可能通过影响胰岛素敏感性以及胰岛β细胞功能导致GDM的发生,其在临床上可能可作为GDM诊断的候选标志物。

     

    Abstract:
      Objective  To explore the value of peripheral blood lncRNA-p3134 in diagnosing gestational diabetes mellitus (GDM).
      Methods  A total of 82 pregnant women with GDM were selected as GDM group, and 116 non-GDM pregnant women were selected as control group. The expression levels of lncRNA-p3134 in peripheral blood of the two groups were detected; the correlations between the expression of lncRNA-p3134 and insulin resistance index (HOMA-IR), insulin sensitivity index (ISOGTT), islet β cell function index (HOMA-β) and the occurrence of GDM were analyzed. The effect of lncRNA-p3134 expression on the occurrence of GDM and the role of lncRNA-p3134 in the diagnosis of GDM were analyzed.
      Results  The relative expression level of lncRNA-p3134 in the GDM group was significantly higher than that in the control group (P < 0.05). LncRNA-p3134 expression was negatively correlated with ISOGTT and HOMA-β (P < 0.05), and positively correlated with HOMA-IR and GDM (P < 0.05). After adjusting confounding factors, lncRNA-p3134 expression was an independent factor affecting the occurrence of GDM (P < 0.05), and the higher the relative expression level of lncRNA-p3134 was the higher the risk of GDM would be. Receiver operating characteristic (ROC) curve analysis showed that the area under the curve (AUC)of lncRNA-p3134 for GDM diagnosis was 0.853, sensitivity and specificity were 75.0% and 83.0%, respectively.
      Conclusion  The expression level of lncRNA-p3134 in GDM is higher than that of non-GDM, which may lead to the occurrence of GDM by affecting insulin sensitivity and islet β cell function, and it may be used as a candidate marker for the diagnosis of GDM in clinical practice.
