经皮椎间孔镜脊柱系统技术治疗腰椎间盘突出症合并神经根管狭窄的疗效观察

Effect of percutaneous transforaminal endoscopic surgical system technique in the treatment of lumbar disc herniation combined with nerve root canal stenosis

  • 摘要: 目的 比较经皮椎间孔镜脊柱系统(TESSYS)技术与经椎间孔入路腰椎椎体间融合术治疗腰椎间盘突出症合并神经根管狭窄的临床效果。 方法 将110例腰椎间盘突出症合并神经根管狭窄患者分为TESSYS组和椎体融合组,每组55例。比较2组治疗效果,手术前后疼痛严重程度、腰椎功能、运动神经和感觉神经传导速度,手术相关指标及并发症发生情况。 结果 TESSYS组患者术中出血量、手术操作时间及术后住院时间均优于椎体融合组,术后优良率高于椎体融合组,差异有统计学意义(P<0.05)。TESSYS组治疗后疼痛视觉模拟评分法(VAS)评分、Oswestry功能障碍指数(ODI)评分低于椎体融合组,日本骨科协会(JOA)评分高于椎体融合组,感觉及运动神经传导速度快于椎体融合组,差异均有统计学意义(P<0.05)。2组患者并发症发生率比较,差异无统计学意义(P>0.05)。 结论 经皮椎间孔镜TESSYS技术疗效显著,可明显缓解患者术后疼痛,改善腰椎功能及下肢神经功能。

     

    Abstract: Objective To compare the clinical effect of percutaneous transforaminal endoscopic surgical system(TESSYS)technique and transforaminal lumbar interbody fusion in the treatment of lumbar disc herniation combined with nerve root canal stenosis. Methods A total of 110 patients with lumbar disc herniation complicated with nerve root canal stenosis were divided into TESSYS group and lumbar interbody fusion group, with 55 patients in each group. The treatment effect, and pain severity, lumbar function, conduction velocity of motor nerve as well as sensory nerve before and after operation of two groups were compared. At the same time, the operative related indexes and rate of complications of two groups were compared. Results Intraoperative blood loss, operative time and postoperative hospital stay were significantly better, and the excellent and good rate of TESSYS group after operation was significantly higher than that of lumbar interbody fusion group(P<0.05). After treatment, the Visual Analogue Scale(VAS)score and Oswestry Disability Index(ODI)score of the TESSYS group were significantly lower, the Japanese Orthopaedic Association(JOA)score was significantly higher, and the conduction velocity of sensory and motor nerves was significantly faster than those of the lumbar interbody fusion group(P<0.05). There was no statistically significant difference in the incidence of complications between the two groups(P>0.05). Conclusion Percutaneous TESSYS technique has a distinct curative effect, which can significantly relieve postoperative - pain, and improve lumbar function and nerve function of lower limbs.
