气管导管拔除时呼气末七氟烷体积分数预测腹腔镜术后躁动的价值

Value of end-expiratory sevoflurane volume fraction at extubation in predicting agitation after laparoscopic surgery

  • 摘要: 目的 探讨老年腹腔镜手术拔除气管导管时呼气末七氟烷体积分数预测术后躁动的价值。 方法 将109例老年患者分为躁动组35例和非躁动组74例。采用 Pearson相关性分析拔管时呼气末七氟烷体积分数与拔管时间的相关性,并通过受试者工作特征(ROC)曲线探讨其预测术后躁动的价值。 结果 躁动组拔管时呼气末七氟烷体积分数高于非躁动组,但拔管时间短于非躁动组,差异有统计学意义(P<0.05)。Pearson相关分析显示,躁动与拔管时呼气末七氟烷体积分数呈正相关(r=0.369, P=0.027), 拔管时间与拔管时呼气末七氟烷体积分数呈负相关(r=-0.345, P=0.039)。拔管时ROC曲线结果显示,曲线下面积(AUC)为0.935, 当最佳临界值取0.17%时,敏感性为82.9%, 特异性为90.5%。 结论 老年腹腔镜手术患者拔管后躁动与拔管时呼气末七氟烷体积分数具有相关性,拔管时呼气末七氟烷体积分数能预测和评估拔管后躁动情况,该分数越高表示拔管后躁动发生率越高。

     

    Abstract: Objective To explore the value of end-expiratory sevoflurane volume fraction at extubation in predicting postoperative agitation in elderly patients with laparoscopic surgery. Methods A total of 109 elderly patients were divided into agitation group(n=35)and non-agitation group(n=74). Pearson correlation was used to analyze the correlation between the volume fraction of end-expiratory sevoflurane at extubation and extubation time, and its predictive value of postoperative agitation was explored by receiver operating characteristic curves(ROC). Results The volume fraction of end-expiratory sevoflurane in the agitation group was higher than that in the non-agitation group, but the extubation time was shorter than that in the non-agitation group, and the differences were statistically significant(P<0.05). Pearson correlation analysis showed that agitation was positively correlated with end-expiratory sevoflurane volume fraction(r=0.369, P=0.027), and extubation time was negatively correlated with end-expiratory sevoflurane volume fraction(r=-0.345, P=0.039). The ROC curve results showed that the area under the curve(AUC)at extubation was 0.935. The sensitivity and specificity were 82.9% and 90.5% respectively when the optimal critical value was 0.17%. Conclusion Restlessness after extubation in elderly laparoscopic surgery patients is correlated with the volume fraction of breath sevoflurane at extubation. The volume fraction of exhalation sevoflurane at extubation can predict and evaluate the agitation after extubation, and the higher the score is, the higher the incidence of restlessness after extubation will be.
