非小细胞肺癌组织中活化蛋白-1、多瘤促活化因子表达对患者预后的影响

Effects of expression of activator protein-1 and polytumour activator in tissue of non-small cell lung cancer on prognosis

      目的  探讨非小细胞肺癌(NSCLC)组织中活化蛋白-1(AP-1)、多瘤促活化因子(PEA3)表达情况对患者预后的影响。
      方法  收集75例NSCLC患者肺癌组织及距癌组织>5 cm处癌旁组织石蜡标本,采用免疫组化法(SP)检测肺癌组织及癌旁组织中AP-1、PEA3蛋白表达水平,分析其表达与NSCLC患者临床病理参数的关系,采用Kaplan-Meier法分析AP-1、PEA3表达与NSCLC患者预后的关系,并对NSCLC患者不良预后的危险因素进行Cox回归分析。
      结果  肺癌组织中AP-1、PEA3蛋白阳性表达率高于癌旁组织,差异有统计学意义(P < 0.05)。肺癌组织中AP-1、PEA3蛋白阳性表达与患者吸烟史、分化程度、TNM分期、淋巴结转移及胸膜转移有关(P < 0.05)。肺癌组织中AP-1、PEA3蛋白阳性表达患者5年总生存率均低于阴性表达患者,差异有统计学意义(P < 0.05)。TNM分期高、胸膜转移、肺癌组织AP-1蛋白阳性表达、PEA3蛋白阳性表达是NSCLC患者不良预后的独立危险因素(P < 0.05)。
      结论  肺癌组织中AP-1、PEA3蛋白阳性表达会影响NSCLC患者的预后,对NSCLC预后评估可能具有临床指导意义。

     

    Abstract:
      Objective  To study the effects of activator protein-1 (AP-1) and polytumor activator factor (PEA3) levels in tissue of non-small cell lung cancer (NSCLC) on prognosis of patients.
      Methods  Lung cancer tissues and adjacent tissues more than 5 cm away from cancer tissues of 75 patients with NSCLC were collected. The expression levels of AP-1 and PEA3 in NSCLC lung cancer and adjacent tissues were detected by immunohistochemistry (SP), the relationships between their expressions and clinicopathological parameters of NSCLC patients were analyzed, Kaplan-Meier was used to analyze the relationships between AP-1, PEA3 expression and the prognosis of NSCLC patients, COX was used to analyze the risk factors affecting the poor prognosis of NSCLC patients.
      Results  The positive rates of AP-1 and PEA3 proteins in lung cancer tissues were significantly higher than those of adjacent tissues(P < 0.05). The positive expressions of AP-1 and PEA3 proteins in lung cancer tissues were related to smoking history, differentiation degree, TNM stage, lymph node metastasis and pleural metastasis of NSCLC patients (P < 0.05). The 5-year overall survival rate of patients with positive expressions of AP-1 and PEA3 proteins in lung cancer tissues was significantly lower than those with negative expressions (P < 0.05). The high TNM stage, pleural metastasis, positive expressions of AP-1 and PEA3 proteins in lung cancer tissues were risk factors affecting the poor prognosis of NSCLC patients (P < 0.05).
      Conclusion  The positive expression of AP-1 and PEA3 protein in lung cancer tissues affects the prognosis of NSCLC patients, and may have some clinical significance for the prognosis evaluation of NSCLC.
