心电图缺血分级与急性心肌梗死患者靶血管定位、狭窄程度的相关性研究

Relationship between myocardial ischemia grading diagnosed by electrocardiogram and target vessel location as well as stenosis degree in patients with acute myocardial infarction

  • 摘要: 目的 探讨心电图缺血分级与急性心肌梗死(AMI)患者靶血管定位、狭窄程度的关系。 方法 选取100例AMI患者作为研究对象,根据心电图缺血分级分组,将50例二级缺血患者纳入二级缺血组,将50例三级缺血患者纳入三级缺血组。比较2组临床相关指标水平、靶血管定位、狭窄程度,随访6个月,统计2组主要心血管不良事件。 结果 三级缺血组血清肌酸激酶同工酶(CK-MB)、脑钠肽(BNP)、心肌肌钙蛋白I(cTnI)基线水平、升高幅度均显著高于二级缺血组(P<0.05); 三级缺血组左前降支病变占比显著高于二级缺血组(P<0.05), 左回旋支、右冠状动脉病变占比显著低于二级缺血组(P<0.05), 靶病变数目>1、100%管腔狭窄者占比显著高于二级缺血组(P<0.05); 三级缺血组主要心血管不良事件发生率为26.00%, 显著高于二级缺血组的8.00%(P<0.05)。 结论 二级缺血患者与三级缺血患者的cTnI、CK-MB、BNP、靶血管定位、狭窄程度存在显著差异,心电图缺血分级可为医师判断AMI患者靶血管定位、狭窄程度提供重要的参考依据。

     

    Abstract: Objective To investigate the relationship between myocardial ischemia grading diagnosed by electrocardiogram and target vessel location as well as stenosis in patients with acute myocardial infarction(AMI). Methods A total of 100 AMI patients were selected as research objects, and were divided into two groups. Fifty patients with grade 2 myocardial ischemia were selected as grade 2 myocardial ischemia group, another fifty patients with grade 3 ischemia were selected as grade 3 myocardial ischemia group. Clinical relevant indicators, target vessel position, degree of stenosis were compared, and major adverse cardiovascular events after 6-month follow-up of two groups was recorded. Results The baseline values and increase degree of serum creatine kinase isoenzyme(CK-MB), brain natriuretic peptide(BNP)and the of cardiac troponin I(cTnI)in the grade 3 myocardial ischemia group were significantly higher than those in the grade 2 myocardial ischemia group(P<0.05); the proportions of patients with left anterior descending branch lesion, more than one target lesion, and entire luminal stenosis in the grade 3 myocardial ischemia group were significantly higher, and proportions of patients with left circumflex artery lesion and right coronary artery lesion were significantly lower than those in the grade 2 myocardial ischemia group(P<0.05); the incidence of major cardiovascular adverse events in the grade 3 myocardial ischemia group was significantly higher than that in the grade 2 myocardial ischemia group(26.00% vs. 8.00%, P<0.05). Conclusion There are significant differences in cTnI, CK-MB, BNP, target vessel location as well - as stenosis degree between grade 3 myocardial ischemia and grade 2 myocardial ischemia patients. Myocardial ischemia grading diagnosed by electrocardiogram can provide a powerful reference for physicians to evaluate the location and stenosis of target vessels in patients with AMI.
