甲泼尼龙片对老年间质性肺疾病患者的影响

Effects of methylprednisolone tablets in elderly patients with interstitial lung disease

      目的  探讨甲泼尼龙片对老年间质性肺疾病患者临床体征、气道重塑及纤维化相关指标的影响。
      方法  将98例老年间质性肺疾病患者随机分为观察组和对照组,每组49例。对照组予以抗感染、抗纤维化治疗,观察组在对照组基础上予以甲泼尼龙片治疗。比较2组临床疗效、临床症状及体征消失时间、肺功能用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、第1秒用力呼气容积与用力肺活量的比值(FEV1/FVC)、气道重塑和纤维化相关指标基质金属蛋白酶-3(MMP-3)、基质金属蛋白酶抑制剂-1(TIMP-1)、成纤维细胞生长因子(b-FGF)、转化生长因子-β(TGF-β)及并发症发生率。
      结果  观察组治疗总有效率为85.71%, 高于对照组的67.35%, 差异有统计学意义(P<0.05)。观察组临床症状及体征消失时间均短于对照组,差异有统计学意义(P<0.05)。治疗后, 2组FVC、FEV1、FEV1/FVC均上升,且观察组优于对照组,差异有统计学意义(P<0.05)。观察组血清MMP-3、b-FGF水平低于对照组,差异有统计学意义(P<0.05)。
      结论  甲泼尼龙片应用于老年间质性肺疾病患者,可有效改善其临床症状及体征、肺功能,减轻气道重塑、肺纤维化。

     

    Abstract:
      Objective  To investigate the effects of methylprednylone tablets on clinical signs, airway remodeling and fibrosis in elderly patients with interstitial lung disease.
      Methods  A total of 98 elderly patients with interstitial lung disease were randomly divided into observation group and control group, with 49 cases in each group. The control group was treated with anti-infection and anti-fibrosis therapy, and the observation group was treated with methylprednisolone tablets on the basis of the control group. Clinical curative effect, clinical symptoms and signs disappeared time, lung function forced vital capacity (FVC), forced expiratory volume in one second (FEV1), FEV1/FVC, airway remodeling, as well as fibrosis related index matrix metalloproteinase-3 (MMP-3), tissue inhibitors of metalloproteinase1 (TIMP-1), basic fibroblast growth factor (b-FGF), transforming growth factor-β (TGF-β) and incidence of complications were compared in the two groups.
      Results  The total effective rate of treatment in the observation group was 85.71%, which significantly higher than 67.35% in the control group (P < 0.05). The disappearance time of clinical symptoms and signs in the observation group was significantly shorter than that in the control group (P < 0.05). After treatment, FVC, FEV1 and FEV1/FVC in both groups were significantly increased, and their levels in the observation group were significantly better than those in the control group (P < 0.05). The levels of MMP-3 and B-FGF in the observation group were significantly lower than those in the control group (P < 0.05).
      Conclusion  Methylprednylone tablets can effectively improve the clinical symptoms as well as signs as well as lung function, and reduce airway remodeling and pulmonary fibrosis in elderly patients with interstitial lung disease.
