新型冠状病毒肺炎疫情期间河北省27家三级甲等医院消化内镜开展情况的调查报告

  • 摘要: 目的 探讨新型冠状病毒肺炎疫情对河北省消化内镜诊疗工作的影响。 方法 采用调查问卷表方式,对河北省27家三级甲等医院进行调查,比较新型冠状病毒肺炎疫情期间(2020年1月1日—4月10日)与2019年同期河北省消化内镜诊疗工作开展情况及变化。 结果 2020年新型冠状病毒肺炎疫情期间,消化内镜检查总量同比减少46.0%, 内镜治疗量同比减少40.9%, 以2月降低最为明显。急诊胃镜诊疗总量下降,但急诊胃镜在所有内镜检查中的占比较2019年同期显著增高(2.0% vs. 1.8%, P<0.05)。新型冠状病毒肺炎疫情期间,经内镜逆行胰胆管造影术(ERCP)在内镜治疗中占比较2019年同期显著增高(14.8% vs. 11.4%, P<0.05)。 结论 在新型冠状病毒肺炎疫情期间,河北省消化内镜检查量及治疗量均较2019年同期显著下降,但急诊胃镜和ERCP比率同比增长,最大限度地保障了危急重症消化道疾病患者的诊治。本研究为今后国家医疗系统应急管理提供决策参考。

     

    Abstract: Objective To explore the impact of outbreak of Coronavirus Disease 2019 on diagnosis and treatment works of digestive endoscopy in hospitals in Hebei Province. Methods A questionnaire survey was carried out in 27 grade Ⅲ level A hospitals in Hebei Province, and sitiation and change of digestive endoscopy works were compared between outbreak of Coronavirus Disease 2019 from January 1 to April 10, 2020 and the same period in 2019. Results During the outbreak of Coronavirus Disease 2019, the total amount of endoscopy examinations decreased by 46.0% in 2020, the amount of endoscopic treatment decreased by 40.9%, and the most obvious reduction was observed in February. The total number of emergency gastroscopy diagnosis and treatment decreased, but the proportion of emergency gastroscopy in all the endoscopy examinations increased significantly when compared with that in the same period in 2019(2.0% vs. 1.8%, P<0.05). During the outbreak of Coronavirus Disease 2019, the proportion of endoscopic retrograde cholangiopancreatography(ERCP)in all the endoscopy treatment increased significantly when compared with that in the same period in 2019(14.8% vs. 11.4%, P<0.05). Conclusion During the outbreak of Coronavirus Disease 2019, the amounts of digestive endoscopy examinations and treatment in Hebei Province decrease significantly when compared with those in same period in 2019, but the proportions of emergency gastroscopy and ERCP increase year-by-year, which greatly ensures the diagnosis and treatment of critically ill patients with digestive tract diseases, and provides a decision-making reference for national medical emergency management in the future.
