消化道肿瘤患者癌性相关性贫血的影响因素分析

Analysis of influencing factors of cancer-related anemia in digestive tract tumor patients

  • 摘要: 目的 探讨消化道肿瘤患者癌性相关性贫血的影响因素。 方法 选择消化道肿瘤患者200例为研究对象,分析癌性相关性贫血的发生情况。患者癌性相关性贫血的影响因素采用单因素分析,危险因素采用Logistic多因素回归分析。 结果 200例消化道肿瘤患者中,癌性相关性贫血124例(62.00%), 其中轻度32例,中度64例,重度16例,极重度12例。单因素分析结果表明, 2组性别、体质量指数、吸烟、饮酒及合并基础疾病比较,差异无统计学意义(P>0.05); 2组年龄、TNM分期、Karnofsky功能状态(KPS)评分、低蛋白血症和抗肿瘤治疗比较,差异有统计学意义(P<0.05)。Logistic多因素回归分析显示,年龄>60岁、Ⅲ~Ⅳ期、KPS评分>60分、低蛋白血症和抗肿瘤治疗为影响癌性相关性贫血的危险因素。 结论 消化道肿瘤癌性相关性贫血发生率较高,以轻中度为主,其中年龄>60岁、TNM分期Ⅲ~Ⅳ期、KPS评分>60分、低蛋白血症和抗肿瘤治疗为癌性相关性贫血的危险因素。

     

    Abstract: Objective To investigate influencing factors of cancer-related anemia in digestive tract tumor patients. Methods A total of 200 digestive tract tumor patients were selected as the research objects, and the occurrence of cancer-related anemia was analyzed. The influencing factors affecting cancer-related anemia were analyzed by univariate analysis, and risk factors were analyzed by Logistic multivariate regression analysis. Results Out of 200 patients with digestive tract tumors, 124 cases had cancer-related anemia(62.00%), including 32 mild cases, 64 moderate cases, 16 severe cases and 12 extremely severe cases. Univariate analysis showed that there were no significant differences in gender, body mass index, smoking, alcohol drinking and complicating with basic diseases between the two groups(P>0.05); there were significant differences in age, TNM stage, Karnofsky Performance Status(KPS)score, hypoproteinemia and antineoplastic treatment between the two groups(P<0.05). Logistic multivariate regression analysis showed that patients aged over 60 years, at stage of Ⅲ or Ⅳ, with KPS scoring>60, hypoproteinemia and antineoplastic therapy were risk factors for cancer-related anemia. Conclusion The incidence of cancer-related anemia in digestive tract is relatively high, occurring mainly in mild to moderate patients. Over 60 years of age, Ⅲ or Ⅳ staging in TNM stage, KPS scoring over 60, hypoproteinemia and anti-cancer treatment are risk factors for cancer-related anemia.
