红花黄色素注射液联合低分子肝素钙预防髋关节置换术后深静脉血栓的效果

Effect of Safflower Yellow Injection combined with low molecular weight heparin calcium in prevention of deep vein thrombosis after hip replacement

  • 摘要: 目的 探讨红花黄色素注射液联合低分子肝素钙预防髋关节置换术(HR)后深静脉血栓(DVT)的效果。 方法 选取HR患者120例,随机分为对照组与观察组,每组60例。2组围术期护理相同,对照组术后1 d给予常规低分子肝素钙皮下注射预防DVT, 观察组给予红花黄色素注射液联合低分子肝素钙预防DVT。2组均连续治疗14 d, 术后共随访3个月。比较2组术前及术后3、14 d凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、D-二聚体(D-D)水平,比较2组术前和随访3个月时Harris髋关节评分,观察2组术后DVT发生情况。 结果 2组术后3 d的PT、APTT水平均较治疗前显著下降, D-D水平显著升高(P<0.05)。观察组术后14 d的PT、APTT、D-D水平与术前比较,差异有统计学意义(P<0.05)。与对照组术后3、14 d比较,观察组PT、APTT水平均显著升高, D-D水平显著下降(P<0.05)。2组随访3个月时Harris髋关节评分均较术前显著升高(P<0.05)。观察组随访3个月时Harris髋关节评分显著高于对照组(P<0.05)。观察组术后DVT发生率为1.67%, 显著低于对照组13.33%(P<0.05)。 结论 红花黄色素注射液联合低分子肝素钙能有效改善HR术后血液高凝状态,促进髋关节功能恢复,预防DVT发生。

     

    Abstract: Objective To investigate the effect of Safflower Yellow Injection combined with low molecular weight heparin calcium in prevention of deep vein thrombosis(DVT)after hip replacement(HR). Methods A total of 120 patients with HR were randomly divided into control group and observation group, with 60 cases in each group. Both groups were conducted with the same perioperative nursing. The control group was given low molecular weight heparin calcium subcutaneously to prevent DVT at 1 d after operation, and the observation group was given safflower yellow injection combined with low molecular weight heparin calcium to prevent DVT. Both groups were treated for 14 days and followed up for 3 months. The levels of prothrombin time(PT), activated partial thromboplastin time(APTT)and D-Dimer(D-D)were compared between the two groups at the time points of before operation, 3 days and 14 days after operation. Harris hip joint scores at the time points of before operation and 3 months after follow-up were compared between the two groups. The mcidence of DVT of two groups were observed Results The levels of PT and APTT in both groups were significantly lower than those before treatment, and the level of D-D was significantly higher than that before treatment(P<0.05). There were significant differences in PT, APTT and D-D levels before treatment and 14 d after treatment in the observation group(P<0.05). Compared with the control group, the levels of PT and APTT at 3 and 14 days after operation in the observation group increased significantly, and the level of D-D decreased significantly(P<0.05). Harris hip score at 3 months - after follow-up was significantly higher than that before operation in both groups(P<0.05). Harris hip score at 3 months after follow-up in the observation group was significantly higher than that in the control group(P<0.05). The incidence rate of postoperative DVT in the observation group was1.67%, which was significantly lower than 13.33% in the control group(P<0.05). Conclusion Safflower yellow injection combined with low molecular weight heparin calcium can effectively improve the hypercoagulable state of blood, promote the recovery of hip joint function and prevent DVT.
