体质量指数正常的多囊卵巢综合征不孕患者性激素与非特异性炎性因子、免疫因子水平的相关性分析

Correlation between sex hormones and non-specificinflammatory factors, immune factor levels in peripheral blood of normal body mass index patients with polycystic ovary syndrome complicated with sterility

      目的  探讨体质量指数(BMI)正常的多囊卵巢综合征(PCOS)不孕患者性激素与非特异性炎性因子、免疫因子水平的相关性。
      方法  选择符合纳入标准的72例PCOS患者作为观察组,同时采用分层抽签法从同期健康体检的健康女性中抽选50例作为对照组。分别测定性激素卵泡生成激素(FSH)、黄体生成激素(LH)、雌二醇(E2)、睾酮(T)、非特异性炎性因子C反应蛋白(CRP)、干扰素-γ(IFN-γ)、白介素-4(IL-4)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平和Th1、Th2细胞含量,并比较其表达水平(或含量)的差异,计算LH/FSH和Th1/Th2比值。采用Pearson相关系数分析性激素与炎性因子、Th1/Th2的相关性。分析观察组患者炎性因子与FSH、LH、E2和T的相关性。
      结果  与健康对照组比较,观察组性激素指标E2、LH、T、LH/FSH均显著升高(P < 0.05), 炎性因子CRP、IFN-γ、TNF-α、IL-6水平显著升高(P < 0.05), 免疫功能指标Th1含量和Th1/Th2比值显著升高(P < 0.05); 2组FSH和IL-4含量变化无显著差异(P>0.05)。进一步Pearson相关系数法分析显示,观察组性激素指标E2、LH/FSH、T表达水平与CRP、IFN-γ、TNF-α、IL-6及Th1/Th2比值均存在显著正相关(P < 0.05); 但E2、LH/FSH、T表达水平均与IL-4水平无显著相关性(P>0.05)。
      结论  BMI正常的PCOS患者体内可能存在较强的慢性非特异性炎症反应和以Th1/Th2比值增大为标志的免疫调节失衡,这些均可能与患者体内性激素功能紊乱有密切关联性。

     

    Abstract:
      Objective  To investigate the correlation between sex hormones and non-specific inflammatory factors, immune factor levels in peripheral blood of normal body mass index (BMI) patients with polycystic ovary syndrome (PCOS) complicated with sterility.
      Methods  Seventy-two patients with PCOS who met the inclusion criteria were enrolled as observation group. At the same time, 50 healthy women from healthy physical examinations were selected as control group by stratified sampling. The levels of sex hormonesestradiol (E2), luteinizing hormone(LH), follicle-stimulating hormone(FSH), testosterone(T), non-specific inflammatory factorsC reactive protein (CRP), interferon-γ(IFN-γ), tumor necrosis factor-α(TNF-α), interleukin-4(IL-4), interleukin-6(IL-6)and Th1 and Th2 cells were determined and the difference of their levels was compared, and the ratio of LH/FSH, Th1/Th2 were calculated. The Pearson correlation coefficient analysis was used to analyze the correlation between sex hormones and non-specific inflammatory factors, Th1/Th2. The correlation between inflammatory factors and levels of FSH, LH, E2, and T.
      Results  Compared with the control group, the levels of sex hormones such as E2, LH, T and LH/FSH in the observation group increased significantly (P < 0.05), and the expression levels of inflammatory factors such as CRP, IFN-γ, TNF-α and IL-6 as well as percentage immune function index such as Th1 and Th1/Th2 increased (P < 0.05), but there were no significant differences in FSH and IL-4 levels between the two groups (P>0.05). Further Pearson correlation coefficient analysis showed that there was a positive correlation between the expression levels of sex hormones including E2, LH/FSH, T and CRP, IFN-γ, TNF-α, IL-6 and ratio of Th1 to Th2 in the observation group(P < 0.05). However, there was no significant correlation between E2, LH/FSH and T expression levels and IL-4 level (P>0.05).
      Conclusion  Chronic non-specific inflammatory responses and immunoregulatory imbalances marked by increased Th1/Th2 ratio may be presented in normal BMI patients with PCOS, which may be closely related to sexual hormonal dysfunction in patients.
