中西药配合纤维支气管镜冲洗治疗肺脓肿的效果观察

Efficiency observation of traditional Chinese and westernmedicines combined with fiberoptic bronchoscope flushing in treating patients with pulmonary abscess

      目的  观察中西药配合纤维支气管镜冲洗治疗肺脓肿的效果。
      方法  将肺脓肿患者120例随机分为中西药配合纤维支气管镜冲洗治疗组(配合治疗组)60例与单独中西药洗治疗组(单独治疗组)60例。比较2组患者的临床疗效、临床症状及体征缓解时间、住院时间、动脉血气分析指标、肺功能指标、不良反应发生情况。
      结果  配合治疗组总有效率93.3%, 显著高于单独治疗组的76.7%(P < 0.05)。配合治疗组体温恢复正常时间、咳嗽咳痰缓解时间、肺部啰音消失时间、脓腔消失时间、住院时间均显著短于单独治疗组(P < 0.05)。2组患者治疗后的血氧饱和度(SaO2)、动脉血氧分压p(O2)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)均显著高于治疗前(P < 0.05), 且配合治疗组上述指标均显著高于单独治疗组(P < 0.05)。配合治疗组不良反应发生率3.3%, 显著低于单独治疗组20.0%(P < 0.05)。
      结论  中西药配合纤维支气管镜冲洗治疗肺脓肿效果较单独中西药治疗更好。

     

    Abstract:
      Objective  To observe the therapeutic effect of traditional Chinese and western medicine combined with fiberoptic bronchoscope flushing in treating patients with pulmonary abscess.
      Methods  A total of 120 patients with pulmonary abscess were randomly divided into combined treatment group (traditional Chinese and western medicine combined with fiberoptic bronchoscope flushing) and single treatment group (traditional Chinese and western medicine), with 60 cases in each group. The clinical efficacy, remission time of clinical symptoms and signs, hospitalization time, arterial blood gas analysis indexes, pulmonary function indexes and adverse reactions were compared between the two groups.
      Results  The total effective rate of the combined treatment group was 93.3%, which was significantly higher than 76.7% of the single treatment group (P < 0.05). The recovery time of body temperature, relief time of cough and sputum, disappearance time of lung rale, disappearance time of pus cavity and hospitalization time in the combined treatment group were significantly shorter than those in the single treatment group (P < 0.05). After treatment, the blood oxygensaturation (SaO2), arterial partial pressure of oxygen p(O2), forced vital capacity (FVC), forced expiratory volume at first second (FEV1) in both groups were significantly higher than those before treatment (P < 0.05), and the above indexes in the combined treatment group were significantly higher than the single treatment group (P < 0.05). The incidence rate of adverse reactions in the treatment group was 3.3%, which was significantly lower than 20.0% in the treatment group (P < 0.05).
      Conclusion  Traditional Chinese and western medicine combined with fiberoptic bronchoscope flushing is better than single application of traditional Chinese and western medicine in treatment of patients with pulmonary abscess.
