米力农注射液治疗高血压性心脏病伴心力衰竭的效果及对运动耐受性的影响

Effect of milrinone injection for hypertensive heart disease patients with heart failure and its influence on exercise tolerance

      目的  探讨米力农注射液治疗高血压性心脏病伴心力衰竭的效果及对运动耐受性的影响。
      方法  将本院收治的94例高血压性心脏病伴心力衰竭患者按照随机数字表法分为对照组和观察组,各47例。对照组给予常规降血压、抗心力衰竭治疗,观察组在对照组治疗基础上给予米力农注射液。对比2组治疗效果,治疗前后血清脑钠肽(BNP)、心肌肌钙蛋白I(cTnI)水平,采用Yaeger心肺功能测试系统评估治疗前后运动耐受性,并记录患者不良反应。
      结果  观察组有效率显著高于对照组(P < 0.05); 治疗后,观察组血清BNP、cTnI水平低于对照组(P < 0.05); 治疗后,观察组运动时间、最大心率、最大摄氧量、最大运动负荷均大于对照组(P < 0.05); 观察组总不良反应发生率与对照组差异无统计学意义(P>0.05)。
      结论  米力农注射液可提高高血压性心脏病伴心力衰竭治疗效果,减轻心肌受损,增强患者运动耐受性。

     

    Abstract:
      Objective  To explore the effect of milrinone injection in the treatment of hypertensive heart disease patients with heart failure and its influence on exercise tolerance.
      Methods  A total of 94 patients with hypertensive heart disease and heart failure admitted to our hospital were enrolled in the study. According to the random number table Method, they were divided into control group and observation group, with 47 cases in each group. Control group was given conventional treatment such as antihypertensive and anti-heart failure therapies, and observation group was given milrinone injection on the basis of control group. The effects and levels of serum brain natriuretic peptide (BNP) and cardiac troponin I (cTnI) before and after treatment were compared between the two groups. The Yaeger Cardiopulmonary Function Test was used to evaluate the exercise tolerance before and after treatment, and the adverse reactions were recorded.
      Results  The effective rate in observation group was higher than that in control group (P < 0.05). After treatment, the serum BNP and cTnI levels in observation group were lower than those in the control group (P < 0.05). After treatment, the exercise time, maximum heart rate and maximum oxygen consumption and maximum exercise load in observation group were greater than those in control group (P < 0.05). There was no significant difference in the incidence of total adverse reactions between two groups (P>0.05).
      Conclusion  Milrinone injection can improve the treatment efficacy of hypertensive heart disease patients with heart failure, reduce myocardial damage and enhance exercise tolerance.
