风心病慢性房颤患者左右心房组织纤维化程度及TGF-β1的mRNA表达差异的研究

Differentiation in extent of fibrosis and TGF-β1 mRNA expression between left and right atria in patients with chronic atrial fibrillation and rheumatic valve disease

  • 摘要: 目的 探讨风湿性心脏病(风心病)伴慢性心房颤动(房颤)患者左、右心房组织纤维化程度及转化生长因子β1(TGF-β1)的mRNA表达是否存在差异.方法 选取接受瓣膜置换的风心病慢性房颤患者40例,风心病窦性心律患者10例.术中分别收取左、右心耳组织各约100 mg.通过苦味酸天狼猩红染色法对心房组织中Ⅰ、Ⅲ型胶原容量分数(CVF-Ⅰ, CVF-Ⅲ)进行半定量分析;应用半定量逆转录-聚合酶链反应(RT-PCR)技术,检测心房组织中TGF-β1的mRNA表达水平.结果 ① 与风心病窦性心律组(窦律组)相比,风心病房颤组(房颤组)患者左房内径显著扩大(P<0.05), 左、右心房组织TGF-β1的mRNA表达、CVF-Ⅰ及CVF-Ⅰ/ CVF-Ⅲ比值明显增加(P<0.05), CVF-Ⅲ变化不明显(P>0.05).② 窦律组患者左心房与右心房TGF-β1的mRNA表达、CVF-I及CVF-Ⅰ/CVF-Ⅲ比值无明显区别(P>0.05); 而房颤组患者左心房较右心房组织中TGF-β1的mRNA表达、CVF-I及CVF-Ⅰ/CVF-Ⅲ比值明显增加(P<0.05).③ 相关分析发现无论在左心房还是右心房, TGF-β1的mRNA表达水平与心房组织CVF-I呈正相关(左: r=0.786, P<0.05; 右: r=0.858, P<0.05).结论 在风心病慢性房颤过程中,左心房组织纤维化程度及TGF-β1的mRNA表达较右心房更加明显,心房组织TGF-β1的mRNA表达增高可能是心房纤维化的分子机制之一.
